With disease progression and higher L-dopa dose requirements, the vast majority of patients start to experience reduced and variable responses to medication, known as motor uctuations. A series of scales has been identi ed in the medical literature that are designed to measure motor uctuations. Unfortunately, clinimetric properties of the majority of these scales, questionnaires, and diaries have not been analyzed in depth, or have achieved poor results. In this chapter, we will describe scales, questionnaires, and diaries evaluating motor uctuations and review their validity, pros, cons, and recommendations for use. Following the guidelines set by the Movement Disorder Task Force on PD Rating Scales, "Recommended" scales are the Wearing OFF-19 and Wearing OFF-9 questionnaires and the Hauser Parkinson's Disease Diary and the CAPSIT-PD ON-OFF Diary. Whereas the MDS-UPDRS shows promise, it has not been su ciently used in published studies to reach this designation; it will probably be recommended in the future.
