Knowledge of the mechanisms of metabolism and elimination of these drugs is crucial when selecting an AED in epilepsy patients with liver disease. Loss of hepatocytes and disruption of liver blood ow may alter the metabolism of some AEDs. Hypoalbuminemia and impaired metabolism by cytochrome P-450 and glucuronyl transferase enzymes may increase free AED levels (Lacerda et al., 2006). The type of hepatic disease may also preferentially a ect some enzyme systems; for example, CYP3A4 is a ected more in hepatocellular dysfunction, and CYP2E1 is a ected more in cholestasis. Unlike renal failure, which can be quanti ed, liver disease is not easily quanti able, so adjustments in dosing are more di cult. Fortunately, even moderate liver dysfunctio
