What is Physiologic Effects Associated with Intramedullary Reaming about?

Reamed intramedullary nailing has lead to a wide spectrum of use, even for proximal and distal fractures. Intramedullary reaming has local and systemic effects. Biologic alterations include changes in blood supply, increased intramedullary pressure and advantageous effects on bone healing. The impact of reaming techniques and evolution of reamer designs on the generation of complications are discussed. If used in a less cautious fashion, intramedullary reaming can cause complications. Among the local effects are the generation of heat and enhanced intramedullary pressure and secondary pulmonary embolization. Therefore, the choice of patients to be selected for early intramedullary nailing should be done carefully. Technical tricks to reduce the fat liberation include careful reaming techniques (little compressive force, adequate drill speed, sharp edges) and the use of reaming systems (RIA reaming with additional suction and irrigation).
