The subject of this book is an important question. How is it that a good number of countries, each with extremely diverse histories and economies, managed to improve their average life expectancy to levels approaching those of the most advanced countries between c. 1890 and 1960, when they were neither relatively rich, nor benefiting from modern scientific medicine? In 1985 the Rockefeller Foundation published a seminal study of the phenomenon of Good Health at Low Cost achieved during the postwar era in the four diverse states of China, Sri Lanka, Costa Rica and Kerala; and in 1986 John Caldwell published a justifiably much-cited article, which argued that each of the latter three's histories before 1945 had endowed them with unusual social capabilities for adopting low-cost preventive health: egalitarianism, including substantial female autonomy; dedication to education; an open political system; and a civilian society without rigid class distinctions (Caldwell, p. 182).
