This study examined the role of operant conditioning in chronic pain by reinforcing 30 chronic back pain patients and 30 healthy controls for increased or decreased reports of pain intensity. Both groups showed similar learning rates, but chronic pain patients had slower extinction of verbal and cortical pain responses and prolonged muscle tension. The results suggest chronic pain patients are more susceptible to operant conditioning factors, which may contribute to maintaining chronic pain.
