Mr. J, a 51-year old male, presented with shortness of breath, chest pain, and hypotension and was found to have an acute pulmonary embolism based on imaging findings of right heart strain on echocardiogram and filling defects in the pulmonary arteries on CT angiogram; further imaging with abdominal CT found left renal vein thrombosis and lung scan showed perfusion defects consistent with pulmonary embolism.
