What is Novel Ratio of Renal Doppler Index for the Detection of Renal Disease about?
Purpose: Resistive Index (RI) obtained from the Doppler pattern of intrarenal arteries is frequently used for the monitoring of renal diseases. However, recent studies have shown that RI is substantially influenced by the elasticity of the systemic vascular bed rather than by specific renal pathology. We calculated a novel ratio of renal RI and RI of the spleen to correct the influence of the systemic Doppler pattern. Material & Methods: 102 patients were examined with renal Doppler sonography. Fifty patients had renal CKD III-IV, 52 patients were scanned because of hypertension with normal renal function (eGFR $60 ml/min). RI was obtained in intrarenal arteries and in the first branche of the spleen arteries. A ratio of RI-kidney/RI-spleen was tested as a potential diagnostic sign for renal damage. Regression analysis was performed for age, eGFR, proteinuria, blood pressure and pulse pressure. Results: Mean renal RI was 0.6360.21 in 102 patients and mean RIratio was 1.1160.11. Regression analysis showed significantly rising RI-ratio with falling eGFR (R 5 0.392; p,0.001). With a ratio-limit of 1.10, ROC-analysis showed sensitivity and specificity of 70% and 71% retrospectively for
