A 52-year-old man presented with abdominal pain and was suspected of having perforated appendicitis or a tumor of the appendix. Examination of the removed appendix found it enlarged to 7 cm in length and filled with a gelatinous mass. Microscopic examination revealed infiltration by inflammatory cells and the protozoan parasite Blastocystis hominis in the submucosal layer, a parasite typically found in the intestines but rarely reported in tissue sections. This case presents an unusual finding of B. hominis
