Foscarnet/ganciclovir/sotrovimab Lack of efficacy and off-label use: case report An approximately 8-month-old boy (AD) exhibited lack of efficacy during treatment with foscarnet and ganciclovir for CMV infection. Subsequently, he received off-label treatment with sotrovimab for COVID-19 [all dosages not stated and not all routes stated]. The boy (AD) had severe combined immunodeficiency secondary to complete DiGeorge syndrome (cDGS). His newborn screen revealed absent T cell receptor excision circles (TRECs). At 6 weeks of age, he presented with hypocalcaemic seizures. CMV was acquired postnatally in the period before he presented and at initial workup was detectable in his blood and CSF. He started receiving treatment with IV ganciclovir and foscarnet dual therapy. Due to his severe combined immunodeficiency (SCID), he also received treatment with replacement immune globulin [IVIG] and prophylactic cotrimoxazole [bactrim] and fluconazole.
