Purpose Ischemia reperfusion injury (IRI) is known to impair early graft function following heart transplantation. Metabolomic alterations in the myocardial microenvironment after restoration of blood circulation are of significance because they may be targets to treat and prevent IRI. Therefore, we investigated the early metabolic changes in the recipient plasma after heart transplantation and their relationship to cardiac allograft outcome. Methods Targeted metabolomic profiling of plasma samples from 83 human cardiac allograft recipients were examined at 0, 1, 6, and 24h after transplantation. We used bioinformatic and statistical techniques to identify significantly changed metabolites, pathways associated with them, and their relationship to cardiac allograft outcomes. Results We observed a total of 39 metabolites that showed significantly altered levels over time in graft recipients after early reperfusion (Figure 1 A). These included amino acids, monosaccharides, and fatty acids that are involved in aminoacyl-tRNA and arginine biosynthesis as well as in butanoate metabolism.
