Optimal sedation in prone position preserving spontaneous respiratory efforts while the gastroenterologist is sharing the airway with the anesthetist is a major challenge during endoscopic retrograde cholangiopancreatography (ERCP). This prospective single blinded randomized entropy-guided trial from Indira Gandhi Medical College, Shimla, India, compared the entropy-guided efficacy of combination of etomidate-ketamine (n = 30) to dexmedetomidine-ketamine (n = 30) in patients undergoing ERCP . The authors found that etomidate-ketamine combination resulted in faster sedation onset, more stable periprocedural hemodynamics, faster recovery from sedation and better endoscopist satisfaction than dexmedetomidine-ketamine combination. This study is expected to be of practical clinical utility to endoscopists performing ERCP.
