defect in FA oxidation. Intermediates of glycolysis and the pentose phosphate pathway are similarly reduced, as is expression of GLUT1. However, maintenance of tissue pyruvate content suggests an alternate source of pyruvate, e.g., lactate. Elevated PDH activity in failing tissue, likely occurring because of markedly decreased PDK4 protein, indicates increased flux of pyruvate into mitochondria, promoting pyruvate oxidation and perhaps competing for anaplerotic pyruvate carboxylation. The latter may explain the pronounced reductions in TCA cycle intermediates despite no change in acetyl-CoA. The polyol pathway appears to be activated in failing human hearts, evidenced by elevated tissue fructose and sorbitol levels. The phospholipid and lysoglycerophospholipid content of failing human hearts is greatly increased in both failing tissue and plasma. Nucleotide synthesis pathways are also substantially reprogrammed, in particular a marked decrease in adenosine metabolism.
