Fatal lymphocytic myocarditis and lack of efficacy: case report A 79-year-old man developed fatal lymphocytic myocarditis during treatment with nivolumab for metastatic melanoma. Further, he exhibited lack of efficacy of prednisolone and aspirin while being treated for the lymphocytic myocarditis [routes not stated]. The man, who had a 2 year history of melanoma of the left foot, presented with a nodular lesion in lungs and lesion in the liver. His medical history was significant for arterial hypertension and permanent atrial fibrillation. Previously, he underwent tumour resection. During the presentation, he was diagnosed with metastatic melanoma. He started receiving nivolumab 240 mg every 3 weeks. Within 1.5 months, he received three courses of the treatment. During the fourth course, he was admitted with well-being deterioration for 6 days. He reported severe headaches, loss of appetite, weakness and shortness of breath. Physical examination did not reveal significant abnormalities. CT scan excluded pulmonary embolism, pleural effusion, stroke and CNS metastases. He had an exacerbation of symptoms and chest pain.
