What is “calm After The Storm”: Cardiac Sympathetic Denervation Of An Electrical Storm. about?

Background: Cardiac sympathetic denervation (CSD) has been shown to reduce implantable cardiac defibrillator (ICD) discharges significantly in patients with ventricular electrical storm (VES). CSD is useful in polymorphic ventricular tachycardia (VT) when other procedures have failed. CSD involves thoracoscopic or open surgical removal of the lower half of the T2-T4 left thoracic ganglia. Case: A 71-year-old male with Dilated Cardiomyopathy an ICD placed for primary prevention presents to the ER with generalized weakness, dizziness, and fatigue after ICD discharges on more than 40 occasions in the last two hours before his arrived, all appropriate after device interrogation. The patient had been admitted three times in the last month for appropriate discharges related to VT in the setting of anxiety and other adrenergic stimulating situations treated with lidocaine and amiodarone. Decision-making: During the current presentation, VT (Fig. ) was refractory to antiarrhythmics, intubation, and sedation, causing hemodynamic instability. After a lengthy discussion with the Heart Team, ablation therapy was not considered feasible due to it polymorphic nature. Therefore, Left CSD was carr
