What is Characterization Of Screen Failures In Patients Evaluated For Transcatheter Tricuspid Valve Intervention (triselect-study) about?

Background: Transcatheter tricuspid valve intervention (TTVI) has increased treatment options for tricuspid regurgitation (TR). However, many patients are declined for TTVI and the characteristics of this population are unknown. Methods: 547 patients were evaluated for severe TR between 1/2016-12/2021 in 3 centers in the US and Germany. Clinical records and echocardiograms were used to assess health-status and right ventricular (RV) and TV morphology. Initially, only T-TEER and TTV annuloplasty were the technologies available with TTV replacement available in 1 center after 08/2019. Results: Median age was 80 years and 60% were female. Due to NYHA class <2, 29 (5%) were not eligible for TTVI. 318 (58%) were amenable for TTVI while 200 (37%) were deemed unsuitable because of clinical futility (CF) in 41 (8%) and anatomical/technical criteria in 159 (29%). Causes of CF were severe pulmonary hypertension, frailty, limitation of prognosis due to comorbid disease. In those declined due to anatomy, severe RV or TV annular enlargement were independent causes of screen failure (Figure 1). 51 screen failed for TTVI due to anatomy underwent cardiac surgery and 14 could be treated with TTV re
