Multiple Sclerosis (MS) has been recognized for more than 100 years, yet its etiology remains unknown. Both laboratory and clinical observations suggest that an autoimmune process, a slow virus infection, or some combination of the two is likely to be implicated in the pathogenesis of MS". Although the evidence for an immunopathologic process underlying MS is indirect, it has become somewhat more compelling in the last few years. The core problem in MS, in my opinion, relates to the nature of the elusive MS antigen, be it virus or a nervous tissue component. This remains completely unknown and until this problem is solved, formal proof of the cause of MS is unlikely to be forthcoming. Certain facts concerning MS are known, however, which will ultimately have to be incorporated into any coherent theory of the disease. Included are the inflammatory nature of the core pathologic process in brain, a genetically determined predisposition to develop the disease as revealed by histocompatibility antigen profile studies, and the immunocyte abnormalities which are the subject of this paper.
