A 24-year-old woman with AIDS and tuberculosis was admitted to a hospital in South Africa with progressive cough, dyspnea, and wasting. Laboratory tests showed very low CD4 counts and positive sputum smears for acid-fast bacilli. A chest radiograph found cavitary lesions in both lungs. She was eventually diagnosed with extensively drug-resistant tuberculosis complicating her HIV/AIDS infection.
