What is In Reply to Drs. Oymak and Onal about?
To the Editor: We read with great interest the article by Aizer et al. (1). In this study, the authors analyzed 214 patients with prostate cancer and found that patients with larger prostates (.50 cm 3 ) are at higher risk for the development of severe acute genitourinary toxicity when treated with intensity-modulated radiation therapy for prostate cancer. In fact, the effect of prostate volume on genitourinary toxicity has been vigorously investigated for prostate brachytherapy, but studies of the effect of prostate volume on toxicity related to external-beam radiotherapy are relatively scarce. For this purpose, this study is well thought out, in that it focuses on this issue. However, we have a few questions to the authors about the study design and the results. The authors chose a cutoff of volume of 50 cm 3 because of data indicating that patients with a prostate volume greater than 50 cm 3 have a 3.5-fold higher likelihood of moderate to severe urinary/voiding symptoms. We know of two other studies that have different cutoff values (43 cm 3 and 35 cm 3 ) (2, 3).
