This is a case of a 42 y/o who presented with recurrent left flank pain. Work-ups revealed a mass at the pancreatic body, cystic mucinous epithelial neoplasm by FNAB, and an ovoid lesion attached to the left adrenal gland, to consider metastasis. Objectives of this video are: to present a case of simultaneous laparoscopic transabdominal adrenalectomy and spleen-preserving subtotal pancreatectomy using same-port placement; and to present the use of fluorescence imaging using ICG in vascular angiography, identification and preservation of splenic vessels. Patient was placed in right lateral decubitus position, right arm hyperxtended & left arm on an arm rest, left side elevated at 60 degrees. 4-port placement was used. Laparoscopic transabdominal left adrenalectomy was first performed. Utilizing the same port placement, patient's position turned slightly to 45 degrees, proceeded with spleen-preserving subtotal pancreatectomy. Identification and preservation of splenic vessels were done using ICG fluorescence. Laparoscopic ultrasonography was done for tumor localization and checking of vascular relations.
