A 48-year-old man presented to the emergency room with a 10x10 cm laceration wound on his left hand after his hand was struck by another vehicle while resting out of the window of his car. Examination revealed an open fracture of the left distal humerus with exposed tendon and bone. Exploration of the wound identified a 2 cm thrombus in the left brachialis artery 10 cm from the elbow.
