What is The Association between Total Serum Bilirubin and Kernicterus Spectrum Disorder in Newborn Infants: A Systematic Review and Meta-Analysis about?
## Background Total serum bilirubin (TSB) is used in the management of neonatal jaundice despite its unclear association with major outcomes. We evaluated the association between TSB and kernicterus spectrum disorder (KSD) in neonates in this systematic review and meta-analysis (PROSPERO number: CRD42019137027). ## Methods We searched PubMed, EMBASE and CENTRAL to 30 April 2021 without language restriction. Two authors independently selected cohort studies that evaluated TSB and KSD, and extracted data using CHARMS checklist, assessed risk-of-bias (RoB) using QUIPS tool and rated certainty-of-evidence using GRADE adapted for prognosis studies. We pooled adjusted odds ratios via generic inverse variance methods (random-effect) using RevMan 5.4. Surprisingly few studies provided suitable data on the association between TSB and KSD, and the data was heterogeneous. As such, findings of such an association in high-risk infants and not in general population should be interpreted with caution. In assessing the risk of KSD, certain demographic and clinical risk factors should be considered alongside TSB. The association between hyperbilirubinemia and attention deficit and autistic spectrum
