Revenue officials must requisition the new TIN cards from the Accountable Forms Division and track issuances using monthly accountability reports. 3. Lost, damaged or cancelled TIN cards must be reported on special forms and accountability remains with the authorized requisitioner until cards are issued, cancelled or transferred.
