The broad clinical use of G-CSF stands in strong contrast to the relatively poor understanding of its biology and pathophysiology. As a mediator formed under various conditions, G-CSF represents a prime biological response modifier. Its effects as a growth factor in haematopoiesis exceed by far the role of a neutrophil recruitment signal, since it also induces proliferation and mobilisation of other leukocyte populations and progenitors on a longer term. Combined with its function as a growth factor, G-CSF is described to exert actions as a cytokine on mature immune cells in the periphery. Neutrophils are primed in their bactericidal properties, whereas G-CSF is predominantly downregulating inflammatory responses by reducing proinflammatory cytokine formation in activated monocytes and macrophages. In addition to increasing peripheral lymphocyte numbers, G-CSF appears to have an indirect modulatory effect on lymphokine release and adaptive immune responses.
