This document describes a case report of a 64-year-old man presenting with a rapidly growing perianal mass that was initially thought to be a thrombosed hemorrhoid. Excision of the mass and lymph nodes revealed a diagnosis of mantle cell lymphoma. Although gastrointestinal lymphoma is common, primary mantle cell lymphoma localized to the perianal region is rare.
