What is CA 19-9 as a Predictor for the Response and Survival in Advanced Pancreatic Cancer Patients Treated with Chemoradiotherapy about?

Materials/Methods: We followed 173 patients who underwent classic pylorus preserving or total PD from 1/1994 to 12/2006. The majority of patients received adjuvant 5-FU based CRT at JHH. Of these 118 (68.8%) recurred and were used in this analysis. Margin status, tumor grade, tumor size, perivascular, and perineural invasion were evaluated. Recurrences were discriminated as locoregional or distant. Recurrence-free survival was calculated from date of surgery to date of first recurrence or date of last follow-up. Other factors such as gender, age at time of surgery, race, and presence of other co-morbidities were evaluated. Logistic regression was used to determine factors associated with locoregional recurrence (local, regional, or locoregional with distant metastases) compared to distant mets without loco-regional recurrence. Results: Median age of all patients was 63 years. Median RT dose was 50 Gy and median number of positive nodes was 3 out of a median of 18 total nodes sampled. A total of 57.6% patients were male and 67.0% underwent pylorus-preserving PD or total pancreatectomy. Patterns of recurrence were: local (19.5%), regional (7.6%), distant metastases (63.6%), and locor
