S Prednisolone Hepatitis B virus reactivation: case report A 70-year-old man developed hepatitis-B virus (HBV) reactivation during treatment with prednisolone for idiopathic thrombocytopenic purpura (ITP).The man had history of chronic hepatitis B (CHB) infection and ITP. He received prednisolone 5 mg/day [route not stated] for previous 3 years. He was followed-up regularly for CHB and had normal ALT and negative hepatitis B e-antigen (HBeAg). He presented with vomiting and fever for 1 day. He tested positive for Omicron variant of SARS-CoV-2. He was isolated in a negative pressure ward at a hospital in Taiwan. Initial biochemical analysis revealed abnormal ALT of 268.0 IU/L and bilirubin 2.0 mg/dL, and HBeAg was positive. HBV DNA titer was high at 5 42 113 IU/mL. Baseline HBeAg and ALT levels 6 months and 1 month prior to the COVID-19 infection, respectively. For COVID-19, he started receiving off-label treatment with molnupiravir. After 5 days, he was discharged.
