Hypertensive retinopathy is damage to the retina caused by high blood pressure. It has both acute and chronic forms. Chronically, it progresses through vasoconstrictive, exudative, and sclerotic phases, causing arteriolar narrowing, arteriovenous nicking, microaneurysms, and silver wiring. Acutely, it can cause retinal hemorrhages, cotton-wool spots, serous retinal detachment, and optic disc edema. Treatment focuses on carefully lowering blood pressure to prevent further damage and vision loss.
