Hypertension and its sequelae are a major cause of morbidity and mortality in the United States. Hypertension affects black Americans disproportionately compared with white Americans, is earlier in onset, and is associated with more severe end organ damage. Recent studies have suggested that patients with essential hypertension have a deficit in the endothelium-derived nitric oxide system during normal basal conditions and during stimulation with acetylcholine. However, this possibility remains controversial because methacholine-induced arterial relaxation was noted to be preserved in these patients.
