This case report describes a 25-year-old woman in her third pregnancy with two previous cesarean sections who presented with a huge left ovarian mucinous cystadenoma measuring 40x30x15 cm and weighing 20 kg. She underwent an emergency cesarean section to deliver a healthy baby followed by a left salpingo-oophorectomy to remove the cyst.
