What is Thoracolumbar Spine Trauma about?

The thoracolumbar region has a unique anatomy which influences both the extent and management of trauma. Thoracolumbar trauma is evaluated using the McAffee classification, Dennis classification, Allen/ Ferguson scale, Thoracolumbar Injury Classification and Severity Scale (TLICS), and AO thoracolumbar fracture classification. Patients should acquire imaging, undergo the ATLS primary survey as well as a motor/sensory/ reflex exam to determine extent of neurologic damage. Nonoperative management includes bracing and physical therapy. Operative management is most commonly done via a posterior approach. Medications and bracing are commonly used to manage osteoporotic vertebral fractures. Vertebroplasty is not recommended by AAOS guidelines while kyphoplasty is a potential option. INTRODUCTION/HISTORY/DEFINITIONS/ BACKGROUND An estimated 75% to 90% of spine fractures involve the thoracolumbar region, 26% of which result in neurologic injury. The prevalence and clinical implications of thoracolumbar spine trauma makes it particularly important for clinicians to understand optimal treatment strategies for each injury pattern.
