disease extension and localization. The objective of this study is to evaluate the influence of FDG-PET on the target definition in head and neck cancer. Materials/Methods: From February 2003, at the Radiation Therapy Department of Reggio Emilia, in collaboration with the Nuclear Medicine and Medical Physic Departments, the FDG-PET imaging has been introduced into radiation therapy planning. To date, 42 patients with head and neck epidermoid cancer underwent PET scan. The images are taken the same day of CT study and in the immobilized treatment position. After the images fusion and registration, the primary and lymph nodal GTVs have been delineated both on CT and PET-CT images. PET information has been used for the definition of high dose volumes only. All the patients have been treated whit PET-CT based IMRT plans. Because of the sensitivity and specificity properties of PET imaging in head and neck cancer, for the final target contour the following rules have been adopted: When FDG-activity was seen outside the CT volume, the final target volume was always the sum of the CT volume plus the PET volume.
