An incident occurred during equipment trials where an assembly backed out of the lift nubbin it was suspended from, causing a dropped object. The immediate solution was to mark nubbins to detect if they start to back out. However, this does not eliminate the hazard. It is proposed that initiatives be made to eliminate the use of loose fit nubbins for all operations by redesigning nubbins or changing procedures to prevent backing out from occurring.
