Purpose/Objective: The majority of ipsilateral breast recurrences after breast conserving therapy for stage I-II breast cancer occur in close proximity to the tumor bed. The necessity of elective whole breast irradiation has therefore been questioned, and multiple institutions have reported excellent results with adjuvant brachytherapy alone in selected patients. We present a novel 3D-conformal radiation therapy (CRT) technique to treat the lumpectomy cavity plus a margin, study the impact of normal respiration on clinical target volume (CTV) coverage, and derive an appropriate planning target volume (PTV) margin. Materials/Methods: This study included a total of 16 patients. Archived CT scans from 7 patients who underwent an Active Breathing Control (ABC) study were used to develop a standardized non-coplanar 4-and 5-field beam arrangement to treat the lumpectomy cavity plus a 15 mm margin (CTV).
