This document reports a case of a massive intracranial immature teratoma found in a stillborn female fetus. During the 25th week of gestation, the mother developed polyhydramnios and intense pelvic pain. A cesarean section was performed and the stillborn fetus weighed 2750g with macrocephaly. The cranial contents weighed 1350g and consisted mostly of a huge tumoral mass replacing the brain.
