Primary sinusitis, or inflammation of the paranasal sinuses, is caused by obstruction of drainage from the sinuses leading to mucus accumulation and infection. Clinical signs include unilateral nasal discharge and lymph node enlargement. Endoscopy often reveals purulent drainage from the maxillary sinus ostia. Chronic cases may require irrigation or debridement of thickened mucosa. A frontonasal bone flap provides access to treat infections of the frontal, maxillary, and ventral conchal sinuses.
