This case report describes an unusual presentation of systemic mastocytosis in a 77-year-old woman. She presented with bilateral neck lymphadenopathy and right pleural effusion. Biopsies of the cervical lymph node and pleura showed extensive mast cell infiltrations, confirming the diagnosis of systemic mastocytosis.
