uncomplicated disease (11.0 SD Ϯ4.1, pϽ0.0008), as was the %PMN (78.4% SD Ϯ6.9 vs. 74.5% SD Ϯ9.9, pϽ0.04). WBC Ͼ15,000 was found in 32.4% of subjects with complicated disease (sensitivity), but only 11.2% of those without (likelihood ratio positive (LRϩ) 2.29). %PMN Ն 80% was found in 50.0% of those with complicated disease, and 36.0% of those without (LRϩ 1.39). Overall, 52.9% of subjects with complicated disease had either elevated WBC or %PMN, and the positive predictive value for having either abnormality was 41.1%. Conclusion: ED patients with complicated acute diverticulitis more often had an abnormally elevated WBC and %PMN than those with uncomplicated disease, but vital signs alone were nondiscriminatory. Based on these data, prompt imaging and early surgical consultation for ED patients with suspected acute diverticulitis having either elevated WBC or %PMN would be justified.
