Objectives: ASD and intellectual disability (ID) are highly heterogenous and prevalent neurodevelopmental disorders (NDDs). Children with NDDs also experience high rates of medical comorbidity, as well as increased frequency and length of pediatric medical hospitalizations. Delirium is a frequent complication of critical medical illness in children, which may be exacerbated by core features of NDDs. In this review, we aim to discuss the current state of literature at the intersection of NDDs and pediatric delirium. Methods: We will review the state of the literature regarding diagnosis, treatment, and management of delirium in pediatric patients with atypical neurodevelopment. We will review clinical vignettes demonstrating NDDs comorbid with delirium and/or catatonia with a focus on factors enhancing or complicating clinical decisions. Results: Loss of a previously achieved developmental milestone has been identified as a common component of pediatric delirium. Sensory sensitivities and communication deficits are both risk factors for delirium and common baseline symptoms of ASD as well as contributing to behaviors easily mistaken for hyperactive delirium.
