Heme is essential for many proteins and is synthesized through a series of steps. Glycine and succinyl CoA condense to form δ-aminolevulinic acid (ALA), which is the first committed step. ALA then goes through several modifications and condensations to form protoporphyrin IX. In the final step, iron is inserted into protoporphyrin IX by the enzyme ferrochelatase to form heme. The liver and bone marrow are major sites of heme synthesis, and defects can cause porphyrias or anemia.
