Carbonic anhydrase inhibitors (CAIs) block carbonic anhydrase enzymes, leading to increased diuresis and metabolic acidosis, and are primarily used to treat altitude sickness, edema, and glaucoma. Acetazolamide is the main CAI, which inhibits bicarbonate reabsorption in the kidneys and reduces aqueous humor and cerebrospinal fluid production. Adverse effects include electrolyte disturbances and CNS symptoms, and CAIs are contraindicated in patients with sulfa allergies and severe liver or kidney disease.
