This case report discusses the treatment of a horizontal root fracture in a 12-year-old girl who presented with a delayed appeal, preventing the repositioning of the coronal fragment. The coronal fragment was treated with mineral trioxide aggregate (MTA) while the apical fragment was left untreated, resulting in complete healing after 24 months of follow-up. The findings suggest that MTA can be an effective treatment option for horizontal root fractures when repositioning is not possible.
