This case report describes a 7-year-old boy who presented with symptoms of juvenile dermatomyositis including rashes, muscle weakness, and joint contractures. While he did not exhibit overt cardiac symptoms, his electrocardiogram showed an abnormally prolonged PR interval. Further investigation including MRI confirmed dermatomyositis and subclinical cardiac involvement. He was treated with immunosuppressants and steroids, and his condition improved including normalization of his heart rate and PR interval.
