The kidney concentrates urine through countercurrent mechanisms in the loop of Henle and vasa recta that create and maintain an osmotic gradient in the renal medulla. The thick ascending limb of the loop of Henle actively reabsorbs sodium without water, raising medullary osmolality. The countercurrent flow in the loop and vasa recta multiply this single effect.
