Lack of efficacy: case report A 58-year-old man exhibited lack of efficacy during treatment with amphotericin-B for Candida parapsilosis [dosage not stated]. The man with end stage kidney disease was on peritoneal dialysis. He had a 4 day history of stomach pain, diarrhoea, and turbid dialysate. Peritoneal dialysate culture yielded Candida parapsilosis. Subsequently, he started receiving flucytosine and IV amphotericin-B. His treatment with IV amphotericin-B was changed to IV anidulafungin due to lack of improvement (lack of efficacy). On day 6, he showed clinical improvement with subsequent dialysate clearance. He received 1 dose of intra-catheter urokinase followed by 4 doses of intra-catheter alteplase in order to clear the biofilm. Later, he was discharged on voriconazole.
