Persistent pulmonary hypertension of the newborn (PPHN) occurs when an infant fails to transition from fetal to newborn circulation, resulting in abnormally high blood pressure in the lungs. Risk factors include difficult delivery and meconium aspiration. This leads to right-to-left shunting of blood and hypoxia. The increased workload on the right side of the heart causes dilation and eventual failure without treatment. Medical management is focused on supporting oxygenation and reducing pulmonary pressure
