This study analyzed primary graft failure (pGF) outcomes in 2497 acute myeloid leukemia patients undergoing unrelated hematopoietic stem cell transplantation with post-transplant cyclophosphamide (PTCy). The incidence of pGF was found to be 5.6%, with 70.9% of those who failed to engraft by day +30 achieving recovery by day +60. Factors such as ANC recovery, Karnofsky performance status, and being in complete remission at the time of HSCT were associated with improved overall survival post-pGF.
