What is Clinical relevance of HITS detection in cardioembolic stroke: P. Batista, V. Oliveira, J. Ferro.Centro de Estudos Egas Moniz, Lisboa, Portugal about?
Background: High intensity transient signals (HITS), are often detected in patients with cardioembolic diseases. They are related to microembolism but their clinical and therapeutical implications remain by large, ## UIlkllOWll. Method: We examined the frequency of HITS, during 30 min TCD simultaneous monitoring of middle cerebral arteries (MCA) in 46 patients (mean age, 61 years) with cardioembolic stroke or TIA, followed at a stroke clinic. For patients on oral Anticoagulation (AC), INRs were obtained on the day of monitorization. Results: Ten (22%) patients presented a total of 46 HITS. They were more often detected and were more frequent in high risk embolic cardiac conditions (P = 0.003), within 1 month of TIA/stroke (P = 0.0003) and in patients with multiple cerebral ischemic events (P = 0.004). Frequency of HITS was not related to the intensity of AC. There was poor agreement between sufficient AC (INR > 2.0) and no HITS detection (contingency coefficient = 0.11). In 10 patients with INR 2. In 7 (15%) of these patients HITS detection through TCD monitoring could lead to a modification in antithrombotic treatment. Conclusion: HITS have some relationship with embolism risk,
