This case report describes a rare case of intraductal tubulopapillary neoplasm (ITPN) of the pancreas found incidentally in a 53-year-old man being evaluated for pancreatitis. Imaging including MRI and endoscopic ultrasound revealed a mass in the pancreatic duct. Biopsies of the mass showed ITPN, a rare premalignant tumor accounting for less than 1% of pancreatic neoplasms. The patient was referred for surgical resection given the malignant potential of ITPN.
