We recently treated such a patient, presenting with NSCLC and a synchronous solitary brain metastasis, and write to highlight to readers the evidence that supports such a radical approach. A 57-year-old man presented with a 3-week history of severe headaches, vomiting and ataxia. He had no past medical history of note other than mild emphysema. A magnetic resonance imaging scan revealed a solitary cerebellar metastasis, which was incompletely resected at craniotomy. The patient's symptoms resolved rapidly and completely after surgery. Histology showed a poorly differentiated carcinoma. He then received postoperative whole-brain radiotherapy (WBRT). A computed tomography of his chest revealed a primary NSCLC of the apical segment of the right lower lobe. He underwent a superior segmentectomy and was found to have a pleomorphic carcinoma, pT 2 N 0. The histological appearances were compatible with those of the brain metastasis. No adjuvant radiotherapy or chemotherapy was given. Subsequently, the patient presented with back pain and was found to have vertebral bone metastases. He died 5 months after his initial surgery.
