Two boys with hypopituitarism, one due to a midline congenital anomaly and a second with idiopathic disease possibly related to perinatal injury (breech delivery), developed elevated serum creatine kinase (CK) activities. The first patient manifested clinical symptoms of muscle pain, tenderness and weakness while the second was asymptomatic. Further evaluation revealed hypocortisolism and secondary adrenal insufficiency. All clinical symptoms and biochemical abnormalities disappeared after initiation of low dose Cortisol therapy. We conclude that elevated CK may serve as a marker of subtle hypocortisolism in hypopituitary children.
